Visitor Authorization Waiver

I affirm by signature below that:

  1. I have read and agree to follow the Institute’s policy on Access to Animal Facility
  2. I acknowledge the health risks of entering the Animal Facility such as animal allergens, bites, scratches, and the presence of biohazardous materials and/or chemicals
  3. I agree that the Orbeli Institute of Physiology, designated escort person, and any associated parties are not liable in the event that I am injured, experience adverse reactions, or harmed during my visit
  4. I will not handle the animals unless I am not approved on IACUC protocol
  5. I will not take any unauthorized photo or video recording
